NEW STUDENT ENROLLMENT

YOU ARE NOT A NEW STUDENT IF YOU ATTENDED McADORY MIDDLE OR McADORY HIGH SCHOOL LAST YEAR.

You must provide the following information at the time of registration to enroll a student in a Jefferson county school.  All information must be verified and deemed acceptable before enrollment is complete.

 

We do NOT ACCEPT folder until ALL items are complete.  

Student:

o Current Alabama Immunization form

o Social Security Card for student

o Birth Certificate, if not parent, proof of Custody

o Withdrawal form from previous school

o   Lease, Deed or mortgage

o   Two (2) utility bills

o   Photo ID or Alabama Driver’s License

If 2nd Party Residence Verification

Homeowner: o Lease, Deed or mortgage

                         o Two (2) utility bills

                         o Alabama Driver’s License

 

Guardian Items: o Current bank statement

                              o W-2 forms,

                              o Pay stubs

                              o Current registration

                              o Any current court docs

                              o Current proof of insurance

JEFFERSON COUNTY BOARD OF EDUCATION RESERVES THE RIGHT TO ACCEPT OR REJECT ANY DOCUMENTATION PROVIDED.
